Office Policies

Communicating with Dr. Hubsch: Primarily face to face in the office and after-hours via phone. Please refrain from sending text messages or emails to communicate with us if your child is sick or if the office is closed. Our phone system has the option to speak directly with the Doctor if you have urgency. In case of emergencies always first call 911.

Medication Refills: Request these during your office visit or during regular office hours, when you can come in and pick up prescriptions. We do not call pharmacies or provide refills after office hours or on weekends. Allow 24 hours for prescription requests. Antibiotics are not prescribed over the phone.

Referrals to specialists: You are responsible for knowing what your insurance requires prior to seeing any specialist. Request your referral at least 2 weeks in advance through our referral department. Once the referral is ready you will be called to pick it up. Once we refer your child, please follow the step by step instructions provided. The list of specialist we provide are the ones we recommend. However, your insurance plan may have other Doctors available.

School forms: Your child must have updated physical and current vaccines to attend school. These forms must be requested in advance and may require a few days of waiting time, especially during high demand season before school entry (July and August of every year).

School Physicals: May, June, and early July are the best times to schedule school physicals although at KWP you can schedule them at any time if is needed or requested by the school specially for migrant families.

After Hours: Our office goes into after-hours mode at 5:30 pm Mon-Fri, 1:00 pm Sat. The phone system provides patients with direct contact with Dr. Hubsch at all times in case any emergency arises when the office is closed. Our philosophy at KWP is to provide personalized care at all times 24/7 all year long.

Release of Records: Our office will provide a diagnostic summary and copies of your child’s vaccine records for free. Any additional copies will cost 50 cents per page for the first 25 pages, then 25 cents for each additional page after that. To release records to Dr. Hubsch the Medical information release form needs to be faxed to your prior Pediatrician. To obtain records from Dr. Hubsch the form needs to be faxed to our office.

New Patients: If you are scheduled for a first-time visit, please make sure you bring immunization records to better assist you and your child. We cannot provide you with school forms at the time of your visit unless you provide these records.
