Health Insurance
Your Health Insurance is a contract between you and your Insurance Company. Please be aware that your insurance company can either pay the full amount billed or have deductibles, percent allowances, co-insurances, and co-payments that must be met by you. Your plan may have a maximum yearly coverage or no good coverage at all. On occasion, if previously arranged due to financial difficulties, a payment plan can be set up.
Any checks returned by your Bank due to NSF will be charged a $20.00 fee and should be paid within 1 week. For your convenience, we accept cash, checks, Visa and Master Card, and All debit Cards with Visa or Master Card logo on it. We accept several HMO and PPO insurance plans as well as Medicaid. Please check our list, if unsure whether we are participants on a specific plan, please call our office or e-mail our office with a request for the information you need.
(Note that we check for e-mails once a day, therefore, is not to be used for any type of emergencies. Allow up to 24 hrs to receive a response)
